What happens when my child reaches the age of three?

• A transition plan must begin when the child reaches the age of two years six months old and a meeting scheduled no later than two years nine months old with your local school district.
• The district that your family lives in will be involved in the transition process.
• The current service provider will be asked to provide a progress report or a comprehensive report along with goals, objectives and recommendations.
