Payment/Funding Information

CLC works with a variety of funding sources:

Medical Insurance	Birth -adolescence	CLC is contracted with PPO, POS and HMO providers. Please see the link to view our current in-network providers. Out-of-Network providers, payment for services is on a fee for service basis. CLC may assist you by billing your PPO plan as an out-of-network provider for reimbursement. Private Insurance

CLC is now accepting insurance; All co-payments, co-insurance, and deductibles MUST be paid at the time of service.

We currently participate in the following Medical Groups or IPA’s for HMO patients:

· Monarch (HMO, Cal Optima, Healthy Kids)

We accept the following Health Plans for PPO/POS patients:

Out of Network Insurance Providers

CLC offers courtesy verification and billing to insurance companies that are out of our network. Payment is due at the time of service and claims are submitted to your insurance on a monthly basis for reimbursement.

We accept Visa, Mastercard, personal checks, and cash.

Intake Process for Comprehensive Programs (including Learning Connection Program and Infant-Parent Program) and Individual ST, OT, PT and ABA services:

Contact CLC to inform us that you are seeking services for your child. At that time, we will provide you with an intake form (also available in PDF form in this section) which must be returned by mail, email or fax. Once we receive the form, you will be contacted by Client Services. The Director of Client Services will contact you to let you know when we may be able to enroll your child. Parents are encouraged to make an appointment to tour the clinic and speak with a senior level staff member about the services as well.

Intake Process for Social Skills Groups:

CLC is currently screening for matches for afternoon social skills groups. Children will be matched according to age and similar goals/needs. Contact CLC to inform us that your are seeking services for your child. At that time, we will provide you with a registration packet (also available in PDF form in this section) which must be returned by mail, email or fax. Once we receive these forms, the Social Skills Program Director sill contact you regarding a screening.
